Heater control panel bulb# / Alternator Bolt?

Howdy - Does anyone have a illumination bulb # for heater control, specifically the fan switch. Also, the guy at O'Reilly checked the voltage from the alternator and said I was missing a bolt or bolts that holds it to the bracket, maybe causing some belt slippage. How many bolts are there?

There is a 12mm head slider bolt and a 14mm head pivot bolt.
