The engine shakes a little on idle . But yea its only in gear that it does it

You might put the font end up on stands, start the engine, and put it in gear.

fuel filter won't affect a low speed load based hesitation unless it's so clogged that it's affecting rate of flow (unlikely).

Mine does the same thing only in first gear and if you change gears to early i think ir might be fuel filter. I knocked the guts out of my catalytic convertor and it helped a little there was a clog in it.

U said its been off the road a while? Bad gas? Maybe some "heat" or other treatment

Make sure the VAF is not sticky and that the connection from it
to the throttle body is air tight and has no cracks that open
up when the motor rocks.
