wont idle when cold

I have a 91 festiva that does not want to idle when its cold. it starts up revs up a little then chugs to a stall. if you give it gas to rev it up it stumbles then revs up fast. after it starts to warm up it will idle low than surge up and down until the car warms up than its ok. could this be the timing is off a tooth? thay have the distributor turned all the way clockwise

check to see if there is a vacuum leak
